3 = 1.25 + c

-1.25 from both sides  

c = 1.75

y = 1/4x + 1.75

perpendicular gradient = -4

y = -4x + c

3 = -4(5) + c

3 = -20 + c

+20 to both sides

c = 23

y = -4x + 23
